Register for School

To begin kindergarten in Illinois, a child should be 5 years old on or before September 1 of the kindergarten year.

Per Illinois law and Board of Education policy, only students who are legal residents of District 15 may attend school in the district. Generally, a student’s residency (proof of living situation) is the same as the person who has legal custody of the student. The person registering the student must submit (1) proof of legal custody and (2) residency (proof of living situation).

Proof of residency (proof of living situation) is required for all incoming Early Childhood students, Kindergarten students, student entering 6th grade, students new to the District, existing families with a lease, and families that are flagged by our third-party residency verification system to complete the registration process. Providing a copy of a current mortgage or lease is a required step for registering or re-registering in District 15.

Parents will be enrolling their child in the regular full-day kindergarten program or have the option to enroll in the Two-Way Spanish Dual Language Program: a two-way language immersion where students are taught literacy and curriculum in both Spanish and English beginning in kindergarten and continuing through the completion of fifth grade. (PLEASE NOTE: The deadline to apply for the Dual Language program is March 14, 2026.)

In addition, a one-way Spanish Dual Language program is offered to students who are native Spanish speakers and qualify for language support services. Eligibility is determined via an English proficiency screening.

All students who are newly registering with the district are required to provide a Birth Certificate or Passport (original with seal). Official birth certificates are issued by the county clerk in the county in which the child was born. Hospital certificates are not acceptable. A certified copy of the child’s birth certificate must be provided within 30 days of enrollment.

Please bring a certified copy of your child’s birth certificate (with seal) or other government document verifying date of birth. An official certificate of every birth should be on file in the locality where the event occurred. These records are filed permanently in a State vital statistics office or in a city, county, or other local office. 

To obtain a certified copy of a birth certificate, write or go to the vital statistics office in the State or area where the event occurred, or visit the National Center for Health Statistics (www.cdc.gov/nchs/w2w/index.htm) for information on where to obtain an official birth certificate.

A Home Language Survey is administered to all students newly registering in the district to help us determine if your child is eligible for language support services. Families will be provided with the Home Language Survey in their spoken language at their home school.

For the 2025-26 school year, in the case of a lease, please note that if the copy of the lease provided expires before August 20, 2025, the family will need to provide an UPDATED lease prior to the start of the school year to remain enrolled for the 2025-26 school year. The lease needs to show that the family lives in District 15's boundaries at the start of the 2025-26 school year.  

Objections Based on Religious Grounds

Parents/guardians who object to examination(s) or immunization(s) on religious grounds are not required to submit their child or wards to either, and must provide a completed Illinois Certificate of Religious Exemption to Required Immunizations and/or Examinations Form signed by the health care provider. Per School Board Policy 7:100, Health, Eye, and Dental Examinations, Immunizations, and Exclusion of Students—Students who are not in compliance with IL student health requirements may be excluded from school.

If applicable, you will need to submit a student transfer form from the previous school attended.

School District 15 charges an annual student fee for book rental/material/technology to defray costs. These fees are payable at the time of online enrollment.

Other fees may include costs for 8th grade graduation gown rental, additional PE clothes, participation in interscholastic sports, participation in instrumental music, optional paid transportation, and student meal plans. There is no charge for the activity bus.

Students who enroll late may pay the annual student fees in the school office when they register.

• Students who qualify for the Free or Reduced-price Meals Program will have student fees & fines waived.

Students’ eligibility for free or reduced-price meals is confidential. If you receive a letter in mid-July to early August that you have been approved by direct certification, you DO NOT need to fill out an application for free or reduced-price meal benefits. Otherwise, A NEW APPLICATION MUST BE FILLED OUT EVERY SCHOOL YEAR. Meal benefits expire in October if you do not complete an application for the current school year.

For your convenience, parents/guardians can pay for school meals and monitor student account balances, transactions, and request low balance notifications online using SchoolCafé:

• Stay informed about your student's lunch account activity.
   
• Be notified on your phone when your student's account is low.
   
• Make secure payments with Visa, MasterCard, or Discover Card. There is a 5% service fee to use SchoolCafé.
   
• Store your credit/debit card for quick and easy repeat use.
   
• Schedule deposits to occur in any frequency you choose.

SchoolCafé allows for access from the web or any mobile device. 

• The SchoolCafé website uses the most up-to-date security available. The online payment plan is an enhancement to the district’s existing pre-pay meal program. Cash or checks will still be accepted.
   
• The SchoolCafé apps for both iOS and Android are now available for download. They're free to install and they make your favorite features even easier to use. Download the SchoolCafé app today on the AppStore or Google Play.

CCSD15 believes that prescription and nonprescription medication should be administered in the home. However, if it is in the best educational and health interest of the child to take prescribed medication during the school day, then a Medication Authorization Form must be completed. This pertains to all medications (prescription, over the counter, and emergency). Medication must be brought to the school by the parent/guardian in the original container. My Ride K-12 is the bus GPS system which can be used by families to help track their child's bus. The My Ride K-12 app will enable you to check the bus stop times and bus stop locations for your student(s), and receive updates directly from the Transportation Department regarding any changes in daily routes. To use it, parents/guardians need to download the mobile app from the App Store or get it on Google Play.

In most cases, students who live more than 1.5 miles from their home school will be provided free bus service through CCSD15. State funding is not provided for students living within 1.5 miles of their schools. These students may ride the bus for a fee provided that space is available and a route already exists. (No stops will be made less than one-half mile from the school building.)

An Application for Paid Transportation will be available on the Transportation webpage for download in September 2026. Please submit a separate application for each student for whom you choose paid transportation. Bus availability will be evaluated in September 2026.

Due to the bus driver shortage, paid ridership continues to be limited and only offered if space is available on an existing bus route. Applications will be accepted in September 2026, but payment will not be accepted until a parent receives notification that their child has been assigned to a bus route. 

If your child is eligible for free bus service, but you do not intend to use the bus or only need transportation before or after school (not both ways), please fill out the Transportation Opt-Out form to indicate for which children and for which bus routes (before and/or after school) bus transportation is not needed.

If a student lives 1.5 miles or more away from school, students are entitled to free bus transportation. Unless a family notifies us that they do not want transportation, their children are placed on a bus route. Some families do not want to use transportation and choose to transport their children to and from school themselves. If we remove students from bus routes that are NOT going to use district transportation, we can reduce bus route times by eliminating the bus stopping at that assigned bus stop.  

If your child(ren) is assigned to a bus route and you wish to continue using district transportation, you do not need to do anything. The OPT OUT form is ONLY for families that do not need all or partial D15 bus transportation. 

Only children who are residents of D15 and attend a D15 school are eligible to be transported from a daycare center within D15 boundaries. If your child's daycare center is located within the school attendance bounadary associated with your home address, is 1.5 or more miles away from your child's home school, or if a hazardous crossing exists as defined by the State of Illinois, then your child may be eligible for district-provided transportation.

District 15 uses ParentSquare, a communication platform designed to streamline school-to-home communication. This platform will serve as our primary method of communication for all district  and school communication.

Before- and after-school care programs are available at District 15’s schools. These programs are operated independently of the school district by the respective park districts or organizations. Visit their websites for general information, pricing, enrollment, and FAQs.  

• Hoffman Estates Park District STAR Program offers before- and after-school care for students who attend Frank C. Whiteley.
   
• Palatine Park District C.A.R.E. Program offers before- and after-school care for students who attend Gray M Sanborn, Jane Addams, Lake Louise, Lincoln, Marion Jordan, Stuart R. Paddock, Virginia Lake, and Winston Campus Elementary. 
   
• Rolling Meadows Park District R.E.C. Program offers before- and after-school care for students who attend Central Road, Kimball Hill, and Willow Bend.
   
• Boys & Girls Clubs of the Northwest Suburbs (BGCNWS) provides after-school programs focused on academic support, character development, and enrichment activities for students at Jane Addams, Lake Louise, Lincoln, and Winston Campus Elementary in Palatine.

Some programs and procedures may differ from school to school, based on the size and location of the building, the grades served, the facilities available, and the programs conducted at the school. Your school administrators will inform you of these additional procedures. The handbook is intended to be a handy resource, but is not meant to take the place of personal communication between home and school. Kindergarten Two-Way Spanish Dual Language Applications are due. All families applying must also have completed Kindergarten registration with the district to be considered. Applications submitted after March 14 will be placed on a waiting list. 

Being considered for the Gifted program? If your student is in Grade 2, 3, or 4 and is scheduled to take the CogAT exam to be considered for the Gifted Program, you will be notified of your student’s eligibility status in March 2026. When do I keep my child home from school? 

• Fever: Keep your child home if they have a fever (temperature of 100°F or greater). Keep your child home for a period of 24 hours after the fever has passed, without fever-reducing medication. 

• Vomiting or diarrhea: Keep your child home for 24 hours after the symptoms have stopped. 
   
• Undiagnosed rash
   
• Communicable diseases (such as strep throat, influenza, chicken pox, measles, COVID, etc.): Keep your child home for a period of 24 hours of antibiotic treatment or after the contagious period for the illness is over. This time frame may be longer for more serious communicable diseases. 
   
• Pink eye (conjunctivitis)
   
• Head lice infestation (live head lice)

Please report illness symptoms completely on the attendance line for your school.

PTA/PTSAs and PTOs units are affiliated with our elementary and/or middle schools in District 15. These units work in partnership with their school's principal and staff, as well as the community-at-large to provide programs and services that enhance the educational environment for all children. They also implement programs that create a sense of community for the families of children enrolled at that school.

A "home school" is the school assignment based on your home address. Please keep in mind that based on programmatic placements, your child may not attend their "home school." See the Bus Tiers and Program Placements tab below for more information. 

Parents/guardians who are not sure which school their children will attend may call the district's Joseph M. Kiszka Educational Service Center at 847-963-3000.

Under special circumstances, and on a very limited basis, parents/guardians may request an Intra-District Transfer of their child(ren) from their assigned home school to another intra-district school. Requests will only be considered upon completing the requested application and providing supporting documentation within the announced timeline. Requests are handled by the District 15 Intra-District Transfer Committee. Each spring, District 15 will announce the dates and deadline to submit requests for consideration for the upcoming school year. For the 2025-26 school year, requests are accepted from May 1-June 30, 2025.

Only children who are residents of District 15 may attend District 15 schools from a daycare center from within the district. The daycare must be within the home school boundaries of the student. Please review the list of local daycare centers and if School District 15 provides transportation from the daycare to the school, or if the daycare center provides transportation to the school.

• If your child’s daycare center is located within the school attendance boundary associated with your home address, your child may be eligible for district-provided transportation if the daycare center is located 1.5 or more miles away from the student’s home school or if hazardous crossings exist as defined by the State of Illinois.
   
• If your child’s daycare center is not located within the school attendance boundary associated with your home address, and you would like the District to consider allowing your child to attend another school where the daycare center is located, you will need to request an Intra-District Transfer on behalf of your child. 
  
  The Intra-District Transfer Committee makes a final determination on the status of each transfer request received. In the event that the Intra-District Transfer Committee approves an intra-district transfer request, parents are responsible for all costs associated with providing transportation for their child to and from school. Requests are approved on a year-to-year basis.
  
  PLEASE NOTE: Space for pay riders is limited and only offered if space permits.

A student’s home school is based on who they live with 51% of the time.
